The Baby's Room Questionnaire 

Directions:  Answer each of the questions below.  For each positive response, record the points listed at the end of each question on a piece of paper.  Add up all the points when finished.

1. Is the garage connected to the home? (5 points)
2. Is the baby's room directly above the garage and/or near a shared wall or entrance? (15 points)
3. Is the room carpeted with synthetic carpet? (20 points)
4. Was the carpet installed within the last six months? (25 points)
5. Is there new furniture in the room? (Award 10 points for each piece)
6. Is there any furniture made of pressed wood in the room? (Award 15 points for each piece)
7. Is the room wallpapered? (15 points)
8. Was the room wallpapered within the last six months? (15 points)
9. Has the room been painted (within the last year) with anything other than nontoxic paint? (25 points)
10. Is the crib mattress made out of synthetic fibers? (20 points)
11. Is the crib bedding made out of synthetic fibers? (20 pints)
12. Are there new stuffed animals and toys in the room? (Award 5 points for each one)
Scoring:
Your baby's chances for a healthy start are the best in a room that has a score of zero.  If you answered yes to all of the questions, your score could be between 200 and 250.  Parents will have to decide what number is an acceptable score for their infant's room.
